Final/Interim Reports
For a one-year grant, the final report is due two months after the grant expiration date. For multi-year grants, interim reports (including income and expense reports for the current fiscal year and for the next fiscal year) are due 30 days after the end of each year of funding, with a final report due two months following the full grant period.
Progress Reports - Renewal Funding
The foundation's review and evaluation process takes three to four months. Grantees interested in receiving uninterrupted funding should submit a letter of inquiry, required progress report (including an income and expense report) four to five months prior to the end of the grant period. Renewal applicants are required to submit a letter of inquiry and progress report for the current grant.

Report Cover Sheet
Following the submission of your report you must also submit a signed report cover sheet. The cover sheet must be signed by an authorized signatory. You may send the signed report cover sheet via email to submit@publicwelfare.org or by fax to 202-265-8851.
